We are dedicated to empowering and uplifting youth, their families, and members of our community through a comprehensive, trauma-informed approach, providing a range of supportive services, including therapy, counseling, life skills education, art therapy, case management, and more via our programs.

Transitional Living Program

Transitional Living Program

TLP offers youth the opportunity to live on our campus for 18 months as they grow and transition into independence, accompanied with up to six months of additional counseling and case management as they prepare for life beyond our walls. We offer tracks for youth and young adults ages 16-22.

The CC offers two tracks for Transitional Living Programs (TLP) that cater to the unique needs of youth

TLP-G is structed and supervised group housing designed for youth ages 16-19.

TLP-S is unsupervised, shared apartments. TLP-S offers less structure and more independence and is suitable for ages 18-22.

The Hub

The Hub

A safe, supervised center for recreation and community open Monday-Friday, 3:00pm-7:00pm. Unhoused youth or youth at-risk of facing housing instability come together at The Hub to discover opportunities and unlock their potential.

Is there a cost associated with your programs?

Thanks to the generosity of our supporters, The CC’s programs are offered at no cost to the clients we serve. Click here to learn more about our programs.

Where do I get started?

Visit our Programs Page and complete the screener to see which program may be a right fit for you! If you meet the criteria for one of our programs, our Intake Coordinator will reach out to you within 1-2 business days. To enroll in our programs over the phone, contact us over at 314-261-6011.

Who is eligible to participate in the Transitional Living Program?

If you are searching for safe and stable housing and are in between the ages of 16-22, this program may be right for you. Give us a call at 314-261-6011 for intake information or visit our Programs page for more information.

What are the Hub’s hours?

The Hub is open Monday-Friday from 3:00pm-7:00pm except for holidays. The Hub is open for youth ages 10-19.

What services are available for CBS participants?

Participants in the Community-Based Services program have access to trauma-informed counseling, expert case management, and educational and employment support. Click here for more information about Community-Based Services.

Complete an intake screener
Have the following information ready:

  1. Youth full name, DOB, address, phone number
  2. If applicable, legal guardian name, address, phone number
  3.  If applicable, school information including grade
  4. Areas of concern
  5. Areas of strength

Intake coordinator will reach out to legal guardians and/or youth to complete the intake process in the next one to two business days
